Title: Children & Youth Intensive Care Management (ICM) #2021-046-1484  

Location:   Care Management, Geneva, NY  

Schedule: Mon. - Fri. 8:00am - 4:00pm or 8:30am - 4:30pm 

  **Flexible schedule currently available, with future opportunities to work from home 1 day per Week**

Salary:      Starting at $17.17/hr. PLUS Based on Education & Experience (to be discussed during phone screening)

 Children & Youth Intensive Care Management (ICM):  With the general supervision of the Children and Youth Care Management Program Manager, works from a trauma informed care perspective to provide assessment, counseling, advocacy, linkage, and coordination of treatment services for children and youth diagnosed with one single qualifying condition such as Severe Emotional Disturbances (SED), HIV/AIDS, Complex Trauma, or two chronic conditions including physical health and substance use disorders, who qualify for Health Home Services as designated by the Department of Health. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.   

  Children & Youth Intensive Care Management (ICM) Qualifications:  BA/BS degree in human services or related field and two (2) years of relevant experience. Must obtain CANS-NY certification within first month of employment. Willingness to provide trainings on Youth Mental Health First Aid and Mandated Reporting preferred.   Must have a valid NYS Driver License as driving is an essential function of this job.
