The Critical Time Intervention (CTI) Nurse will play a vital role in supporting individuals during pivotal transitions in their lives. Provide intensive case management and support to individuals transitioning from institutional care to community-based settings.
Conduct comprehensive medical assessments and monitoring to identify client needs and potential medical conditions/concerns within their scope of practice.
Obtains and records client vital signs and weight as necessary
Develop and implement individualized care plans in collaboration with clients and interdisciplinary teams.
Facilitate access to healthcare, housing, and social services to ensure continuity of care.
Monitor client progress and make necessary adjustments to care plans to address changing needs.
Build and maintain strong relationships with community resources and service providers.
Educate clients and their families on health management, coping strategies, and available resources.
Advocate for client needs within healthcare systems and community services.
Document all client interactions, assessments, and care plans accurately and in a timely manner.
Participate in team meetings and case conferences to ensure coordinated care.
Provide support to clients as needed.
Maintain up-to-date knowledge of best practices in critical time intervention and relevant healthcare regulations.
Collaborate with the Population Health Specialist to address health related issues with a complex population.
Qualifications

Must be a Licensed Practical Nurse (LPN)
Valid driver’s license and reliable transportation.
Flexibility to work occasional evenings or weekends as needed.
Minimum of 2 years of experience in community nursing, behavioral health, care coordination, or related field preferred.
Knowledge of community resources, including medical, behavioral health, housing, and social service systems.
To perform this job successfully, an individual should have knowledge of Microsoft Suite Applications.
Salary starting at $23.07 - 25.64/hour depending on education, credential and experience. $5,000 sign on bonus fully paid after one year of service.
