An Aurora House Care Manager is a professional nurse, who works collaboratively in partnership with comfort home staff; hospice agency professionals, physicians and other disciplines within the comfort care home and the community. This position is per diem and reports to the Care Coordinator.
Professional Requirements:
• RN Preferred / will consider LPN
• A New York State Nursing license in good standing.
• Recent hospice, palliative or home care experience is preferred.
General Duties:
• Provide quality end-of-life care and support to guests, family, and visitors.
• Ensure continuity of care, following an individualized care plan.
• Assist in any way needed to ensure smooth functioning of the household.
• Engage in attitudes and actions compatible with the Aurora House mission and philosophy.
Duties:
• Daily assessment of guest's response to interventions and medications.
• Prepare unit doses of scheduled and PRN medications for a period of 24 hours with safe labeling.
• Contact with overnight aide for night report before 7 a.m.
• Available for “on call” shift of 12 - 24 hrs for each workday.
• Provides on-site care when called for unrelieved symptoms, family distress, volunteer/aid staffing issues, and imminent death, arriving within 30 minutes of call.
• Communicates changes in medical/physical status to comfort home team and community hospice team.
• Collaboratively works with comfort home team and hospice team to implement changes to care plan as needed.
• Maintain accurate medication administration records.
• Organizes and maintains appropriate levels of medication and supplies needed for care and comfort. Works with community team to ensure timely refills.
• Provides education to family, volunteers, and home health aides.
• Follows onsite TOD protocol.
• Ensures safe environment in home by noticing and minimizing hazards such as icy conditions, poor lighting, and needed repairs.
• Other duties as assigned
Physical Requirements:
• Must be able to lift 30 lbs from waist to chest level regularly.
• Must be able to descend and climb stairs on a daily basis.
• Must be able to squat, bend, or reach to retrieve supplies from lower shelves/cupboards located 4 inches above floor level and above head several times daily.
Other Requirements:
• Must have a valid driver’s license and reliable means of transportation.
• Must have phone/cell phone availability for on call responsibilities.
• Must have basic knowledge of MS Office applications (Word, Excel) and Internet resources (e-mail, Google Drive).
